- Marriage Contract
- Annotated Birth Certificate
- Annotated Marriage Contract to show annulment/divorce/court ordered instruction
- Death Certificate of Spouse

Original and photocopy of any of the following for confirmation of Philippine Citizenship:
- valid Canadian Permanent Residency card
- valid visitor visa
- valid work permit

- Original Birth Certificate from the Philippine Statistics Authority (PSA) to support changes in the passport entries, or if old passport has no complete middle name (if applicable)
- For Married Females: Original PSA Marriage Certificate
- Original and photocopy of any of the following for confirmation of Philippine Citizenship: valid Canadian Permanent Residency card; valid visitor visa; valid work permit
- Any of the following valid Philippine-issued ID with photocopy:
- SSS
- GSIS
- LTO Driver’s License
- PRC
- OWWA/iDOLE
- COMELEC Voter’s ID
- PWD ID
- Senior Citizen ID
- School ID (for students);
- Processing fee of CAD 87.00
C. Supporting Documents
In addition to the core requirements for the renewal of e-Passports, the following supporting documents will be further required in the following cases:
- In case of a Lost ePassport
- Original PSA Birth Certificate and photocopy
- For Married Females: Original PSA Marriage Certificate
- Any of the following valid Philippine-issued ID with photocopy:
- SSS
- GSIS
- LTO Driver’s License
- PRC
- OWWA/iDOLE
- COMELEC Voter’s ID
- PWD ID
- Senior Citizen ID
- School ID (for students);
- Affidavit of Loss executed at the Consulate (Fee: CAD 36.25)
- Processing Fee: CAD 217.50
- In case of a Lost Green/Brown/MRP/MRRP/ Passport
- Original PSA Birth Certificate and photocopy
- For Married Females: Original PSA Marriage Certificate
- Any of the following valid Philippine-issued ID with photocopy:
- SSS
- GSIS
- LTO Driver’s License
- PRC

- In case passport has been mutilated or damaged
- Original PSA Birth Certificate and photocopy
- For Married Females: Original PSA Marriage Certificate
- Any of the following valid Philippine-issued ID with photocopy:
- SSS

- LTO Driver’s License
- PRC
- OWWA/iDOLE
- COMELEC Voter’s ID
- PWD ID
- Senior Citizen ID
- School ID (for students)
- Affidavit of Explanation executed at the Consulate (Fee: CAD 36.25)
- Applicant must surrender mutilated/damaged passport
- In case applicant is a Dual Citizen
- Philippine Government-issued ID or Canadian Government-issued ID
- Original Identification Certificate issued by the Philippine Embassy/Consulate or the Bureau of Immigration (BI)
- In case applicant is a Naturalized Filipino Citizen
- Identification Certificate of Naturalization from the Bureau of Immigration (BI)
- In case applicant is a Filipino Citizen by Election
- Affidavit of Election of Philippine Citizenship
- Identification Certificate of Election from BI
- If applicant has been granted citizenship by Act of Legislation
- Certified true copy of the law granting citizenship
- Authenticated Foreign Birth Certificate
