Consular Mortuary Certificate

Note:

Consular Mortuary service is by appointment only. Please book an appointment below link http://www.calendly.com/yycpcg/nonpassport.

A Mortuary Certificate is required for the shipment of remains or urns for repatriation to the Philippines. However, if the deceased is a Filipino citizen at the time of death, a Report of Death (ROD) must be registered. Please refer to the Report of Death for the separate set of requirements and applicable fee.


Requirements

  • Original and two (2) photocopies of Certificate of Death issued by the Vital Statistics Office of Canada
  • Original and two (2) photocopies of Embalming Certificate (if the remains are embalmed) or original and two (2) photocopies of Cremation Certificate (if the remains are cremated)
  • Original and two (2) photocopies of data page of the Passport of the deceased
  • Original and two (2) photocopies of the Medical Certificate of Death
  • Original and two (2) photocopies of Burial or Transit Permit;
  • Original and two (2) photocopies of certificate stating that the casket contains only the body of the deceased, or certification from crematorium stating that the urn contains only the ashes of the deceased; and
  • Flight itinerary of the remains, including the name address and telephone number of the receiving funeral home or consignee in the Philippines; address and phone number and email address of the next of kin in Canada and in the Philippines.

Submission by mail:

  • Originals and two (2) photocopies of the requirements.
  • Bank draft/money order indicating the corresponding service fees payable to the Philippine Consulate General (in exact words and no abbreviation). Personal checks and cash will not be accepted;
  • One (1) self-addressed Xpresspost return envelope; and Applicant’s contact details.

Fees CAD$ 36.25

Payment Option:Fees can be made either in cash, bank draft, postal money order, debit card, or credit card (with 2.4% convenience fee applied for credit card only if the application is submitted in person)


Processing Period

Processing of Consular Mortuary Certificate applications takes seven (7) working days, counted from the day after receipt of the complete application from the mail. Incomplete / non-compliant applications will not be accepted or processed.
