9(E) Visa for Accredited Government Officials, their Families and Attendants
- Applicant’s passport, with at least one blank page, and valid for at least six months beyond the intended length
of stay in the Philippines
- Duly accomplished visa application form, with passport-size photo taken within the last six months
- Note Verbale from applicant’s Ministry of Foreign Affairs
- Authority to issue visa from DFA Home Office
13(A) Immigrant Visa (For Alien Spouses or Children of Filipino nationals)
All applicants for Immigrant Visas must be made in person.
- In the case of a minor under fourteen (14) years of age, the application may be made for him by any
person who, in the opinion of the consular officer, is responsible for his welfare. The minor (not an infant
in arms) shall be required to appear at the consular office for interview, accompanied by the person
applying on behalf of the minor.
The consular officer shall require applicants, whether under the quota or non-quota categories, to accomplish FA Form
No. 3 in triplicate at the Consulate General. Applicants shall submit the following documents:
- Duly-accomplished Immigrant Visa Application Form (FA Form No. 3 (to be uploaded by Ted on website) in 3
original copies)
- Applicant’s valid passport, with at least one blank page, and valid for at least one year from date of
application
- Photocopy of passport data page
- Four passport pictures (2”x2”) signed across the bottom front
- Authenticated FA Form No. 11 (is there a hyperlink for this) or the Medical Examination report including HIV
test. The medical results shall be valid within six (6) months from the date of examination. (Form to be
uploaded)
- Chest X-ray plate (taken within the last six months), to be presented to the Philippine Immigration authorities
at port of entry. The plate must be sealed by the Philippine Consulate General. The Philippine Bureau of
Quarantine now accepts digital versions (contained in compact discs) of X-ray results as an alternative to the
commonly used X-ray films.
- Authenticated Police Clearance from RCMP
- Authenticated Birth Certificate issued by the Vital Statistics Office
- Authenticated Marriage Certificate, for married applicants, from the Vital Statistics Office, if married in
Canada, or from Philippine Statistics Authority (PSA), if married in the Philippines
- Travel document (Passport) of spouse (if married)
- Authenticated Evidence of Financial Support, i.e., letter from the company sponsoring the trip, financial,
assets, certificate from the bank, etc.
- Philippine passport and original copy of birth certificate of wife or husband
- Authenticated Certificate of Canadian Citizenship (for naturalized Canadian citizens)
- Affidavit stating that applicant(s) intends to reside permanently in the Philippines with the supporting
documents such as land titles and other evidence of ownership being disposed of in the country of where he/she
is a citizen/resident
- If the applicant is bringing a motor vehicle, please secure a license to import from the Philippine Department
of Trade Office. Other requirements will apply in this case.
- Other documents deemed necessary by the Consular Officer
- Processing Fee of CAD 217.50 per applicant (non-refundable, cash or bank draft from a local bank)
- One (1) piece 10” X 13” Envelope
- Please make sure to indicate your contact number and email address in your application form. A personal
interview with the Consular Officer may be conducted in-person, if necessary.
13(B) is an immigrant visa issued to a child of alien parents born during the temporary visit abroad of the mother.
Eligibility:
- Mother has been previously admitted for permanent residence in the Philippines.
- The child is accompanying or joining either of his parents in the Philippines.
- The child applies for admission into the Philippines within five (5) years from the date of his birth.
Requirements:
- Canadian Passport valid for at least six (6) months beyond authorized period of stay in the Philippines. If not a Canadian Citizen, applicants must show proof of legal residence in Canada, in addition to the valid Foreign Passport.
- Duly-accomplished application form FA Form No. 3. (Duly notarized by a notary public)
- Six (6) photographs (2″X 2″) with white plain background, signed on the front left side.
- Medical Certification (DFA Form No. 11) duly accomplished by any physician whose signature must be notarized by a notary public. Must be submitted together with chest x-ray in digital image (CD format) and laboratory reports (original and two copies). Medical reports must be valid for at least six (6) months prior to the date of filing of application.
- Police Clearance Certificate taken not more than six (6) months prior to date of filing of visa application.
- Proof of the applicant’s financial capacity (e.g. title of real estate property, investment certificate, bank certificate of existing checking or savings account, or notarized Affidavit of Support from a relative in the Philippines).
- Visa fee of CAD 217.50 (non-refundable, cash or bank draft from a local bank payable to the “PHILIPPINE CONSULATE GENERAL”).
Please submit above documents in triplicate, arranged in the following order/sequence:
- First Set – All original documents.
- Second Set – Original FA Form No. 3 and copies of all other documents.
- Third Set – same as second set.

13© is an immigrant visa issued to a child born subsequent to the issuance of the immigration visa of the accompanying parent, the visa not having expired.
Eligibility:
- Parents immigrant visa remains valid.
- Proof of child’s legitimacy, i.e. child’s birth certificate; parents’ marriage contract.
Requirements:
- Canadian Passport valid for at least six (6) months beyond authorized period of stay in the Philippines. If not a Canadian Citizen, applicants must show proof of legal residence in Canada, in addition to the valid Foreign Passport.
- Duly-accomplished application form FA Form No. 3. (Duly notarized by a notary public)
- Six (6) photographs (2″ x 2″) with white plain background, signed on the front left side.
- Medical Certification (DFA Form No. 11) duly accomplished by any physician whose signature must be notarized by a notary public. 13(D) is an immigrant visa issued to a woman who was a citizen of the Philippines and who lost her citizenship because of her marriage to an alien or by reason of the loss of Philippine citizenship by her husband, and her unmarried child under twenty-one, if accompanying or following to join her.
Eligibility:
- Substantial proof that original entry and admission to the Philippines is lawful.
- Applicant must establish animus revertendi or an intention to return to an unrelinquished residence in the Philippines.
- Applicant has no record of any derogatory information against the applicant in any local or foreign law enforcement agency.
Requirements:
- Canadian Passport valid for at least six (6) months beyond authorized period of stay in the Philippines. If not a Canadian Citizen, applicants must show proof of legal residence in Canada, in addition to the valid Foreign Passport.

13(E) is an immigrant visa issued to a person previously lawfully admitted into the Philippines for
permanent residence, who is returning from a temporary visit abroad to an unrelinquished residence in the
Philippines.
Applications can be filed at the Bureau of Immigration in the Philippines.

13(F) is an immigrant visa issued to the wife or the husband or the unmarried child under twenty-one years of age, of an alien lawfully admitted in the Philippines for permanent residence prior to the date on which the Immigration Act of the Philippines becomes effective and who is resident therein.
13(G) is an immigrant visa issued to a natural born citizen of the Philippines, who has been
naturalized in foreign country. [As amended by Republic Act No. 4376]
Eligibility:
- Proof of being a natural-born citizen of the Philippines (i.e. PSA-issued birth certificate).
- Naturalization papers in a foreign country.
- Applicant has no record of any derogatory information against the applicant in any local or foreign law
enforcement agency.
- Applicant must intend to reside permanently in the Philippines.
Requirements:
- Canadian Passport valid for at least six (6) months beyond authorized period of stay in the Philippines. If not
a Canadian Citizen, applicant must show proof of legal residence in the US, in addition to the valid Foreign
Passport.

done, the applicant may submit his/her application for 13A Visa at the PCG. Please note that a
processing fee of CAD 36.25 will be charged for each document for
authentication by the Consulate.

AUTHENTICATION OF DOCUMENTS :
- FOR ALBERTA-ISSUED DOCUMENTS:
1. Have the document notarized by their local notary public, if the document is issued by a private company (e.g.
bank, employment company, private individuals or office) in Alberta. If the document is issued by any Government
office in Alberta please follow step 2.
2. Send the notarized document for provincial authentication to Edmonton. The document containing the Notary Public’s
original signature and seal can be mailed, brought in person, or delivered by courier to the Deputy Provincial
Secretary’s Office along with a written request for the authentication. There is a fee of CAD 10 for each
certificate provided, payable to the Government of Alberta.

B. FOR SASKATCHEWAN-ISSUED DOCUMENTS:

The document is then stamped with the Great Seal of Saskatchewan and is considered authenticated.
The fee for authentication is CAD 50 cash, cheque or money order payable to the Minister of Finance. Allow three
business days for the authentication process. The requestor may then arrange to pick up the documents in person or
have them mailed. If the requestor cannot pick them up and need the documents immediately, they should include a
pre-paid self-addressed method of returning the documents to them.
