The death of a Filipino citizen in Alberta or Saskatchewan should be reported to
and registered with the Philippine
Statistics Authority (PSA) through the Philippine Consulate General (PCG) in
Calgary. If the deceased is no longer a
Filipino at the time of death, there is no need to file an ROD.
Documentary Requirements
​When reporting/registering the death, the following documents
should be submitted:
- Report of Death Form (4 original duly-filled out forms)

- Use black ink in accomplishing the form.
- Original and four (4) photocopies of the following:
- Proof of Philippine citizenship of the deceased
(valid Philippine passport, and/or
Identification Certificate as dual citizen issued by the Philippine
Bureau of Immigration or any
Philippine Embassy/Consulate)
- Proof of the deceased’s immigration status in
Canada (valid PR card, work/study permit)
- Certificate of Death issued by the Vital Statistics
Office of Canada
- Data page of the Philippine passport of the
deceased
- Embalming Certificate, if the remains are embalmed
- Cremation Certificate, if the remains are cremated
- Medical Certificate of Death (stating the cause of
death)
- Burial or transit permit for shipment of human
remains
- Copy of valid passport of reporting party, and
other documents that may be required by
the consular officer
- Mortuary
Certificate issued by the PCG, if required
- NOTE: The Mortuary Certificate is required
by PH authorities for the shipment
to the Philippines of human remains or cremated remains in
urns. This may be filed separately or
closer to the date of departure to PH.
- Processing fee of CAD 36.25 in postal money order/bank
draft made payable to the “PHILIPPINE
CONSULATE GENERAL CALGARY” (Note: Please use these exact words. Do not
abbreviate. Otherwise, the bank will not
accept your postal money order/bank draft). Do not send cash in the mail.
Personal cheques are not accepted.
Requirements for a Mortuary Certificate
- ORIGINAL and one (1) photocopy of the following documents:
- Death Certificate issued by the Vital Statistics
Office;
- Embalming Certificate, if the remains are embalmed;
- Cremation Certificate, if the remains are cremated.
The certificate should state that
"the human remains have been properly cremated and ashes are placed
in a hermetically sealed urn or a
similar container.”
- Travel itineraries for the remains and the one who
will accompany the remains;
- Original and photocopy of the Philippine passport of the
deceased;
- Medical Certificate of Death;
- Burial or transit permit for shipment of human
remains;
- Certificate stating that the casket contains only the body of the
deceased, or certification from
crematorium stating that the urn contains only the ashes of the
deceased; and
- Processing fee of CAD 36.25.
Please bring the required number of photocopies of documents. The PCG has no
photocopying machine for public use and
photocopying is not part of consular services offered.
Procedure for Application
In-Person Submission
Applicants wanting to submit a ROD in person
should book an appointment at http://www.calendly.com/yycpcg/nonpassport.
Only those
with confirmed appointments will be allowed entry in the PCG.
Below are the step-by-step procedures for in-person submission:
- Book an appointment for a notarial/civil registration service with the PCG
using its Calendly appointment
system: http://www.calendly.com/yycpcg/nonpassport.
- On the date and time of your appointment, please bring the documents stated
in IA., IB., and IC. above.
- Payment Option: Payments should be made in Canadian Dollars either in cash, bank draft, postal money order, debit card, or credit card (with 2.4% convenience fee applied for credit card only). Personal cheques are NOT accepted.
- Self-addressed and stamped unused return envelope that the PCG will use to
mail the ROB to the applicant; and
- Note stating the applicant’s full name, address, email, and phone
number.​
Please note that processing of the documents takes seven (7) business days,
counted from the next business day
following receipt of application, before the documents are returned by mail to
the applicants, provided the
requirements and procedures listed above are complied with. The PCG is NOT
RESPONSIBLE for any delay or loss in the
mail or any other uncontrollable circumstances surrounding the notarization or
authentication of the documents.
It would be best to use an envelope with tracking features so the applicant would
know the status of the delivery of
the documents by Canada Post/private courier service.
For Applications Submitted By Mail
- The ROD Form must be
notarized by a local notary public.
- The notarization by a local notary public must be

- The locally notarized and authenticated ROD and affidavits, together with
return envelope and money order/bank draft payment must be mailed to the PCG
at this address:

- Mailed applications should likewise include the contact details (telephone
and/or email address) of the party
that filed the report in case the PCG needs further information on the
documents submitted.
- Self-addressed prepaid envelope (preferably with tracking features) to be
used in mailing the applicant’s copy
of the ROD/Mortuary
Certificate.​
Applicants must comply with the above requirements. Applications with incomplete
requirements will not be processed
and will instead be returned to the applicant.
Regular processing of ROD takes seven (7)
working days.

Statistics Authority (PSA) after about six months from the date of filing of
said report at the Philippine Consulate
