If receiving survivor pension/death pension from deceased husband, please bring the following:

• SSS number of the deceased member
• SSS number of the pensioner
• Valid passport / SSS Digitized ID
• Address in Canada of the pensioner
• Email address and mobile number in Canada of pensioner
