Registration of Affidavit to Use the Surname of Father (AUSF)

Book an appointment for a notarial service with the PCG at any of these two appointment sites:


For a child born in the Philippines where the mother is currently residing in Alberta or Saskatchewan, Canada, the AUSF shall be notarized and registered at the Philippine Consulate General.

If the mother of the child is in the Philippines but the birth occurred in Alberta or Saskatchewan, Canada, the AUSF shall be notarized and registered at the Local Civil Registry Office of the place of execution of the document in the Philippines.

The Local Civil Registrar’s (LCR) Office or the Philippine Consulate General, whichever is applicable, shall also issue an accompanying Certificate of Registration in relation to the AUSF.

Documentary Requirements for the Submission of the AUSF:

  • Four (4) duly accomplished AUSF by the mother/child/guardian (write the first name, middle name, and last name); AUSF form can be found here.
  • Four (4) photocopies of the birth certificate of the child issued by the Philippines Statistics Authority (PSA) or LCR.
  • Original and four (4) photocopies of the mother’s passport and 1 government-issued ID with the father’s signature.
  • Prepaid return envelope (preferably with tracking number).

Note

The AUSF shall be executed by the mother if the child is aged 0-6 years old, by the child with the mother’s sworn attestation if aged 7-17 years old, and the child himself if aged 18 years old and above.

Fee of CA$36.25 for AUSF and CA$36.25 for the Certificate of Registration. Payment can be made in cash/debit/credit (with a 2.4% convenience fee applied for credit card only) if the application is submitted in person.

Submission by mail:

  • Four (4) AUSF executed by the mother/child/guardian (first name, middle name, and last name must be written) must be notarized by a local Notary Public in Alberta or Saskatchewan and individually apostilled, whichever is applicable.
  • Four (4) photocopies of the birth certificate issued by the Philippine Statistics Authority (PSA) or LCR.
  • Four (4) photocopies of the mother’s passport and 1 government-issued ID with the father’s signature.
  • Prepaid return envelope (preferably with tracking number).

Fee of CA$36.25 for the AUSF and CA$36.25 for the Certificate of Registration in Bank Draft or Postal Money Order, payable to the Philippine Consulate General (in exact words and no abbreviation) if the application is to be submitted by mail.

Processing:

Please note that processing of the documents takes five (5) business days, counted from the next business day following receipt of application, before the documents are returned by mail to the applicants, provided the requirements and procedures listed above are complied with. The PCG is NOT RESPONSIBLE for any delay or loss in the mail or any other uncontrollable circumstances surrounding the registration of the documents.
