Adult Renewal Applicant (18 years old and above)

BASIC REQUIREMENTS FOR ADULT RENEWAL APPLICATION

A. Core Requirements
  1. Confirmed Online Appointment: Please read Passport Booking Guidelines first and book an appointment here: www.passport.gov.ph
  2. Personal appearance
  3. Current ePassport with photocopy of date page
  4. Original PSA-issued documents that will support the change of name:
    • Marriage Contract
    • Annotated Birth Certificate
    • Annotated Marriage Contract to show annulment/divorce/court ordered instruction; and/or
    • Certificate of Death of Spouse
  5. Original and photocopy of any of the following for confirmation of Philippine Citizenship:
    • valid Canadian Permanent Residency card
    • valid visitor visa
    • valid work permit
  6. Processing fee of CAD 87.00 (payments can be made in cash, debit or credit card)
  7. Unused self-addressed envelope, preferably with tracking features, to be used in mailing the new passport to the applicant

Note: If a woman opts to retain maiden name, a PSA-issued Certificate of Marriage is not required.

B. Renewal of Green Passports, Any Older Passports, and Machine-Readable Passports (Maroon)
  1. Confirmed Online Appointment
  2. Personal appearance
  3. Accomplished application form
  4. Passport with photocopy of data page
  5. Original Birth Certificate from the Philippine Statistics Authority (PSA) to support changes in the passport entries, or if old passport has no complete middle name (if applicable)
  6. For Married Females: Original PSA Marriage Certificate
  7. Original and photocopy of any of the following for confirmation of Philippine Citizenship:
    • valid Canadian Permanent Residency card
    • valid visitor visa
    • valid work permit
  8. Processing fee of CAD 87.00
  9. Unused self-addressed envelope, preferably with tracking features, to be used in mailing the new passport to the applicant
C. Supporting Documents

Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship, and lack of travel restrictions, as specified in the following cases:

  1. In case of a Lost VALID/EXPIRED ePassport
    • Original PSA Birth Certificate and photocopy
    • For Married Females: Original PSA Marriage Certificate
    • Police Report
    • Affidavit of Loss executed at the Consulate (Fee: CAD 36.25)
    • Processing Fee: CAD 217.50
    • Note: If a photocopy of the applicant's latest issued passport is not available, on top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.
  2. In case of a Lost Green/Brown/MRP/MRRP Passport
    • Original PSA Birth Certificate and photocopy
    • For Married Females: Original PSA Marriage Certificate
    • Police Report
    • Affidavit of Loss executed at the Consulate (Fee: CAD 36.25)
    • Processing Fee: CAD 130.50
    • Note: If a photocopy of the applicant's latest issued passport is not available, on top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.
  3. In case of renewal of a frequently Lost Passport - TREATED AS A NEW APPLICATION
    • Original PSA Birth Certificate and photocopy
    • For Married Females: Original PSA Marriage Certificate
    • Police Report
    • Affidavit of Loss executed at the Consulate (Fee: CAD 36.25)
    • Processing Fee: CAD 217.50
    • Note: If a photocopy of the applicant's latest issued passport is not available, on top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.
  4. In case passport has been mutilated or damaged
    • Original PSA Birth Certificate and photocopy
    • For Married Females: Original PSA Marriage Certificate
    • Affidavit of Destruction of Passport executed at the Consulate (Fee: CAD 36.25)
  5. In case applicant is a Dual Citizen
    • Natural-born Dual Citizens:
      • Latest-issued Philippine passport
      • PSA-issued Certificate of Live Birth or PSA-issued Report of Birth
    • Dual Citizens who availed of the provisions of Republic Act No. 9225:
      • Latest-issued Philippine passport
      • Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine Embassy/Consulate; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by BI
      • Canadian Issued government ID
  6. In case applicant is a Naturalized Citizen
    • Latest-issued Philippine passport
    • Identification Certificate of Naturalization from the Bureau of Immigration, or Certificate of Naturalization issued by the Special Committee on Naturalization (SCN), or copy of the final Court Judgment or Law granting Philippine citizenship
  7. In case applicant is a Filipino citizen by Election
    • Latest-issued Philippine passport
    • Affidavit of election of Philippine citizenship
    • Identification Certificate issued by the BI
  8. If a married woman applicant wishes to revert to her maiden name based on the following:
    • By virtue of death of the spouse:
      • PSA-issued Certificate of Death or Report of Death (ROD) of spouse or apostillized or authenticated Foreign Death Certificate of foreign spouse with English translation, if applicable
      • PSA-issued Certificate of Live Birth or Report of Birth
      • Latest issued Philippine passport (if available)
    • By virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
      • PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
      • PSA-issued Certificate of Live Birth or Report of Birth
      • Latest issued Philippine passport (if available)
    • For other reasons of reversion:
      • PSA-issued Certificate of Live Birth or Report of Birth
      • Notarized Affidavit of Explanation that includes request for the reversion of maiden name in the Philippine passport or travel document and stating she has not hitherto availed of the reversion
      • Latest-issued Philippine passport or travel document
      • Any existing government-issued valid ID accepted for passport application reflecting the maiden name

Note: This mode of reversion can only be done once.
