Passport Application - Minor Applicant (Below 18 years old)

A. Core Requirements

1. New Applicants

  1. Confirmed online appointment - Please read Passport Booking Guidelines first and book an appointment here: www.passport.gov.ph

  2. Personal appearance of minor applicant and either parent or authorized adult companion (please also refer to Item B.3 and B.4 below)

  3. Original PSA authenticated Birth Certificate / Report of Birth

  4. Passport or valid government-issued ID of either parent showing full name (First Name, Middle Name, Last Name)

  5. For child born in Canada to Filipino parent/s, original and photocopy of Canadian passport of the child

  6. Original and photocopy of any of the following for confirmation of Philippine Citizenship:

    • valid Canadian Permanent Residency card
    • valid visitor visa
    • parent/s’ valid work permit

    Note: Please see Important Information below if you do not have VALID visa, PR Card, or Work Permit

  • Processing fee of CAD 87.00
  • Unused self-addressed envelope, preferably with tracking features, to be used in mailing the new passport to the applicant

2. Renewal

  1. Confirm the online appointment and accomplished application form at www.passport.gov.ph
  2. Personal appearance of minor applicant and either parent or authorized adult companion (please also refer to Item B.3 and B.4 below)
  3. Passport with photocopy of data page
  4. Marriage Certificate is required if only one parent is accompanying the child
    • If parents are unmarried, a Special Power of Attorney (SPA) executed by the Mother will be required if she is not accompanying the child. The SPA must be authenticated by the Department of Foreign Affairs
    • If Special Power of Attorney (SPA) is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
  5. Passport or valid government-issued ID of either parent showing full name (First Name, Middle Name, Last Name)
  6. Proof of filiation to accompanying parent: original Birth Certificate of the applicant (please also see Section B. “Supporting Documents” below)
  7. Original and photocopy of any of the following for confirmation of Philippine Citizenship: valid Canadian Permanent Residency card; valid visitor visa; parent/s’ valid work permit
  8. Processing fee of CAD 87.00

B. Supporting Documents

In addition to the core requirements, the following supporting documents will be further required in the following cases:

  1. In case applicant has No PSA Birth Certificate or Report of Birth yet
    • Parents to first file the child’s Report of Birth (ROB). Please refer to requirements for Civil Registration: Report of Birth at the Consulate’s website for the requirements
    • Submit original parent’s copy of ROB
    • If a photocopy of the applicant’s last issued passport is available, the PSA Birth Certificate will not be required.
  2. In case applicant is not accompanied by the parent/s during the application process
    • Submit a Special Power of Attorney (SPA) executed by the parent/s designating the minor’s companion to assist in the application process. The SPA must be authenticated by the Department of Foreign Affairs (DFA) in Manila if executed by Philippine-based parent/s. If executed abroad, the SPA must be authenticated by the Philippine Embassy/Consulate where the parent/s reside.
    • PSA Birth Certificate (proof of filiation)
    • Passport or valid government-issued ID of authorized adult companion showing full name (First Name, Middle Name, Last Name)
  3. In case applicant is an illegitimate child in custody of the Mother
    • Personal appearance of minor applicant and mother or authorized adult companion
    • PSA Birth Certificate (proof of filiation)
    • Passport or valid government-issued ID of mother or authorized adult companion showing full name (First Name, Middle Name, Last Name)
    • If applicant is not accompanied by mother during the application process, applicant must submit a Special Power of Attorney (SPA) executed by the mother designating the minor’s companion to assist in the application process. The SPA must be authenticated by the Department of Foreign Affairs (DFA) in Manila if executed by Philippine-based parent/s. If executed abroad, the SPA must be authenticated by the Philippine Embassy/Consulate where the mother resides.
  4. If the applicant is an illegitimate child and mother is deceased/absent and father is unknown
    • Personal appearance of minor applicant and adult guardian
    • PSA Birth Certificate (proof of filiation)
    • Valid passport or valid government ID of adult guardian showing full name (First Name, Middle Name, Last Name)
    • DSWD Clearance
    • Affidavit of Guardianship / Order of Guardianship
  5. If the minor applicant is undergoing or has undergone the process of domestic adoption
    • Personal appearance of minor applicant and adoptive parent/s
    • PSA Birth Certificate
    • Valid passport or valid government issued ID of adoptive parent/s showing full name (First Name, Middle Name, Last Name)
    • Court Decree of Adoption
    • If applicant is not accompanied by adoptive parent/s during the application process, applicant must submit a Special Power of Attorney (SPA) executed by the adoptive parent/s designating the minor’s companion to assist in the application process. The SPA must be authenticated by the Department of Foreign Affairs (DFA) in Manila if executed by Philippine-based parent/s. If executed abroad, the SPA must be authenticated by the Philippine Embassy/Consulate where the adoptive parent/s reside.
  6. If the minor applicant is undergoing or has undergone the process of foreign adoption / Inter Country Adoption Board (ICAB) adoption process
    • Personal appearance of minor applicant and ICAB representative
    • PSA Birth Certificate / PSA Certificate of Foundling
    • Endorsement from ICAB
    • Certificate of Child Available for Adoption / Deed of Voluntary Commitment
    • Placement Authority issued by ICAB
    • Certificate for Issuance of Passport issued by ICAB
    • Clearance for Inter-Country Adoption
    • Child Study Report
    • DSWD Clearance
  7. If the applicant is a foundling NOT for adoption
    • Personal appearance of minor applicant and adult guardian
    • PSA Certificate of Foundling
    • Passport or Valid Government issued ID of adult guardian showing full name (First Name, Middle Name, Last Name)
    • DSWD clearance
    • Affidavit of Guardianship
